out treatment, payment, or healthcare operations associated with RxMapper Services (“TPHO”). You further understand that You may set or change Your communication preferences with RxMapper at any time under the “Communication Preferences” in your account.

With this consent, You understand and agree that RxMapper may call Your home number, mobile number, or other alternative number and leave a message on voicemail in reference to any items that assist RxMapper in carrying out TPHO, such as reminders, insurance items, shipment and
delivery of DNA collection kits, health profile updates, and any calls pertaining to my care.

With this consent, You understand and agree that RxMapper may contact You via unencrypted SMS text messages in reference to any items that assist RxMapper in carrying out TPHO, such as reminders, insurance items, shipment and delivery of DNA collection kits, health profile updates,
and any communications pertaining to Your care, including but not limited to text messages exchanged with RxMapper personnel and automated text message campaigns. You acknowledge that you are solely responsible for any charges from your mobile carrier.

With this consent, You understand and agree that RxMapper may contact You via unencrypted email in reference to any items that assist RxMapper in carrying out TPHO, such as reminders, insurance items, shipment and delivery of DNA collection kits, health profile updates, and any
communications pertaining to Your care, including but not limited to emails exchanged with RxMapper personnel and email message campaigns.

Video Conference
With this consent, You understand and agree that RxMapper may communicate with You via video conference to discuss any items that assist RxMapper in carrying out TPHO, such as reminders, insurance items, shipment and delivery of DNA collection kits, health profile updates, and any
communications pertaining to Your care, including but not limited to telehealth consults.
